An upstate New York man has been charged after a flywheel crashed into a motorcoach on the QEW in St. Catharines last week, killing one person and injuring three others, the OPP say.

The collision occurred shortly after 3:30 p.m. last Friday in the Toronto-bound lanes of the highway on the descending side of the Garden City Skyway Bridge.

Ontario Provincial Police said one of a van’s wheels came loose and struck a pickup truck in the opposite lane before smashing through the windshield of a bus.

Images from Chopper24 show a tour bus with its windshield shattered and a minivan with a missing front wheel.

The incident left a 48-year-old Toronto bus passenger dead and sent three other passengers to hospital with serious injuries.

It is unclear how many people were on the bus at the time.

In a statement to CTV News Toronto on Sunday, the OPP said a 45-year-old male minivan driver, a resident of New York State, has been issued a Part III summons under the Provincial Offenses Act for driving an unsafe vehicle . This violation is contrary to Article 84, first paragraph, of the Road Traffic Act. He will appear in court in Welland on September 24.
